All parents/carers are encouraged to complete the form, even if they do not wish their child to receive the vaccination. Submitting a response, including selecting the "Decline" option, will allow the Immunisation Team to update their records and prevent further reminder communications from being sent.

Please note: The consent link will close three days before the scheduled school vaccination session date. Any forms submitted after this deadline may not be processed in time for your child's school session.
