Please see below the letter and information leaflet for the Vision Screening taking place in school on Wednesday 25th February. If you do not wish for your child to participate in the vision screening, kindly e-mail oxl-tr.ncmp@nhs.net including your child’s name, date of birth, school attended, your name and optional information about why you are declining. Please do not ask school personnel to pass on messages in relation to the screening and consent on your behalf.
